By the way, on Nietszsche, I read this fictional book, 'When Nietzsche Wept' by Irvin Yalom, which is a bit cheesy, but it induced me to read Nietzsche's books, 'The Gay Science' and 'Human, All Too Human', and I think the quote is related to the symptoms which plagued him throughout his life...many of which really sounded like TMS to me :). The impression I got was that he thought his symptoms prevented him from leading a settled existence, and he felt this made it possible for him to lead a very monastic life and hence think all these deep thoughts.

I'm an amateur about all this so feel free to correct inaccuracies.
